Fix btc_eth dialog sizing labels and buy-coin index.

Use option index for premium/U and buy-coin, perp underly for hedge size text/margin, so cross-asset no longer mislabels ETH qty as BTC. Co-authored-by: Cursor <cursoragent@cursor.com>
